Special Care Dentistry — Compassionate, Individualized Dental Care in Rockville, Bethesda & DC Metro
Everyone deserves compassionate dental care that meets them where they are. Natural Dentist Associates specializes in special care dentistry for neurodivergent individuals, patients with physical disabilities, and those with complex medical needs throughout Rockville, Bethesda, and the DC metro area.
Over 5 million Americans live with autism spectrum disorder, and millions more have ADHD, sensory processing differences, or physical disabilities that make conventional dental care challenging. At Natural Dentist Associates, we’ve created a dental home where every patient — regardless of ability — receives compassionate, individualized care.
Our all-female professional team specializes in caring for neurodivergent individuals including those with Autism and ADHD, patients with sensory processing disorders, individuals with physical disabilities, those with developmental or intellectual disabilities, and medically complex patients managing multiple health conditions.
What makes our approach unique is the integration of special care expertise with biological dentistry principles. This combination ensures biocompatible materials for patients with compromised immune systems, gentle techniques that minimize stress, and whole-body health support rather than isolated dental treatment.
We’ve designed our practice specifically for sensory-sensitive patients:
Special care dentistry requires collaboration with your entire healthcare team. We coordinate with neurologists, developmental pediatricians, behavioral therapists, occupational therapists, and primary care physicians to understand each patient’s unique needs.
For medically complex patients, we review medications and their oral health impacts, coordinate sedation when needed, and adapt treatment protocols for conditions like bleeding disorders or immune suppression. We partner with caregivers through hands-on training, adaptive equipment recommendations, and dietary counseling tailored to texture preferences and nutritional needs.
Our biological approach is especially important for patients with compromised immune systems or multiple chemical sensitivities. We use biocompatible, toxin-free materials while avoiding mercury, fluoride, BPA, and other potentially harmful substances. This natural approach supports overall health while providing comprehensive dental care that honors each patient’s unique vulnerabilities.

I have been a patient of Dr. Kimberly Baer for 2 years now. She is very caring, kind and gentle. She does not rush and explains procedures and treatments very well. Dentists that take into consideration your overall health and do not use toxins are difficult to find. I live 80 miles away, but I am willing to travel the distance to get this quality of dental care. I highly recommend Natural Dentists Associates.

I’ll admit that I was a bit embarrassed when I first walked into Natural Dentist Associates. My mouth was a total mess: broken, missing teeth, missing crowns, poor gums, all due to long neglect on my part. But Dr. Tipograph and the entire staff gave me nothing but understanding and assurance. After a series of thorough exams, including x-rays, scans, and other imaging, they put together an affordable plan with a mix of crowns, one implant, a very comfortable set of partial dentures, and a night guard. The only problem was that after so many years, I actually had to re-learn how to smile and properly chew my food! I highly recommend them and can’t thank them enough!

This is honestly one of the most pleasant dental experiences I’ve ever had. Very through and informative. Compassionate, very knowledgeable. Dr Baer made me feel at ease and comfortable with treatment options from unresolved issues from previous dental work done at my old dentists

I highly recommend this wonderful dental practice that I have been using for many years. All those I’ve seen here from my wonderful dentist Dr. Kimberly Bear as well as Dr.Brown, Dr. Tipogragh and the dental hygienist Kelsey and Maria the assistant - all are excellent dentists and dental associates who really care about their patients and do excellent work. I know that everyone I refer to this practice will receive excellent care.
Additionally the team has been patiently and beautifully caring for my 99yo mother’s dental needs. She is a retired health professional and very picky. She loves Dr Bear and the practice!

I have been coming to this Dentist practice for 20 years and my kid for 10 years! I trust them and the care is outstanding. I’ve enjoyed the care of Dr Baer and Dr Tippograph. Dr Brown did a fantastic job fixing my son’s broken teeth and was so patient through 3 different appointments to get them as close to as they were. That was amazing! We love Kelsey who does our cleanings. I recently had two fillings, one of which was sooo complicated! Dr Tippograph took her time to minimize pain and make sure I was doing ok and comfortable during the whole procedure, fitting me in their schedule because I was flying overseas. I actually declined to see a closer dentist and drove all the way here. The staff is wonderful and respectful and overall you feel safe with their procedures! The billing is clear as they provide you an estimate of your cost ahead of time as they’re an out of network provider and it is an important consideration. Nevertheless, we moved to a different county but we come here because of the great service and value in this practice!
READ MOREWe provide comprehensive accommodations including sensory-friendly treatment rooms with adjustable lighting and reduced noise, extended appointment times for gradual desensitization, wheelchair accessibility with specialized positioning equipment, communication supports like visual schedules and picture boards, and behavioral management using positive reinforcement. Our environment features comfortable waiting areas with minimal overstimulation, customizable treatment rooms, equipment modifications to reduce anxiety-provoking sounds, and flexible scheduling for optimal cooperation times. We coordinate closely with families and caregivers to understand each patient’s triggers, preferences, and communication style, creating individualized care plans that evolve with changing needs and ensure positive dental experiences building trust for future visits.
We use specialized approaches including gradual desensitization with pre-visit tours and sensory introduction, tell-show-do techniques with simplified language and visual aids, positive reinforcement systems with preferred rewards, and calming sensory supports like weighted blankets and noise-canceling headphones. Our all-female team is trained in neurodivergent communication styles and behavioral strategies, creating predictable routines that reduce anxiety. We offer flexible sedation options when anxiety prevents care, coordinate with behavioral therapists to align with each patient’s existing strategies, and use gentle biological dentistry techniques minimizing invasive procedures. Parents and caregivers actively participate in treatment, providing comfort and familiar communication methods that help patients feel safe and supported throughout their dental visit.
Yes, when properly managed by trained professionals who understand complex medical needs. We carefully evaluate each patient’s medical history, current medications, and specific conditions before recommending sedation. Options range from nitrous oxide for mild anxiety to oral conscious sedation or general anesthesia for patients unable to cooperate due to severe anxiety, intellectual disabilities, or physical limitations. We coordinate closely with physicians, anesthesiologists, and medical specialists to ensure safe sedation protocols accounting for medication interactions, seizure disorders, cardiac conditions, or respiratory issues. Our biological approach emphasizes using the minimal sedation necessary while maintaining patient safety and comfort. For medically complex patients, we may recommend hospital-based treatment with anesthesiologist supervision. Safety protocols include pre-sedation medical clearance, appropriate monitoring equipment, emergency preparedness, and post-procedure recovery monitoring.
Home oral care requires creative adaptations tailored to each individual’s abilities and challenges. We provide hands-on caregiver training including positioning techniques for safe effective brushing, demonstrations with adaptive equipment like three-sided toothbrushes or electric brushes reducing manual dexterity requirements, and desensitization strategies for patients with oral aversions. Use visual timers showing brushing duration, incorporate favorite songs or videos creating positive associations, and establish consistent routines at the same times daily. For patients with swallowing difficulties, use minimal toothpaste and suction if available. Consider dietary modifications reducing sugar and sticky foods, and use xylitol products between brushing. We recommend specific adaptive equipment, provide samples of biocompatible products suitable for sensitive patients, and troubleshoot challenges with individualized solutions. Regular professional cleanings become even more important when home care is limited.
Our team has specialized training in special care dentistry beyond standard dental education. This includes coursework in developmental disabilities understanding autism spectrum, Down syndrome, cerebral palsy, and intellectual disabilities; behavioral management techniques for non-cooperative or anxious patients; medical management of complex conditions including seizure disorders, cardiac issues, and bleeding disorders; communication strategies for non-verbal or limited-verbal patients; positioning and transfer techniques for patients with physical disabilities; and sedation protocols for patients unable to tolerate treatment awake. Our biological dentistry training adds expertise in biocompatible materials crucial for immunocompromised patients, whole-body health connections, and minimally invasive techniques. We maintain continuing education in special needs care, coordinate regularly with other specialists, and participate in professional organizations focused on special care dentistry ensuring our team stays current with best practices for accommodating diverse abilities.
We use multiple communication methods tailored to each patient’s abilities and preferences. Visual supports include picture communication boards showing dental procedures and instruments, visual schedules illustrating appointment sequence, social stories preparing patients for visits, and demonstration using models before treatment. For patients using augmentative and alternative communication (AAC) devices, we learn their communication system and collaborate with speech therapists to integrate it into dental care. We use simplified language with concrete terms, give patients time to process information, and watch for non-verbal cues indicating discomfort or anxiety. Some patients communicate through caregivers who interpret signals and preferences. We validate communication attempts, provide choices when possible empowering patients, and establish yes/no response systems patients can use during treatment. Building trust through patient, respectful communication helps many initially non-responsive patients become more cooperative and engaged over time.
The first visit focuses on building trust and comfort rather than completing extensive treatment. We recommend scheduling a consultation without treatment pressure, allowing your child to meet our team, explore the office, see equipment, and practice sitting in the dental chair. Bring comfort items from home, use social stories or visual schedules to prepare them, practice opening their mouth and sitting in a chair at home, and maintain positive language avoiding scary words. Communicate your child’s specific triggers, calming strategies, and communication methods to our team. We’ll create an individualized plan starting with simple tasks like counting teeth or gentle cleaning, building gradually to more complex treatments as trust develops. Many parents worry their child “won’t cooperate,” but our specialized training helps us work with patients at all ability levels. Success comes from patience, consistency, and celebrating small progress rather than expecting perfection.
Many medications for disabilities have significant oral health side effects requiring proactive management. Anti-seizure medications often cause gum overgrowth making oral hygiene difficult and increasing periodontal disease risk. Medications for ADHD, anxiety, or behavioral issues frequently cause dry mouth reducing saliva’s protective effects and increasing cavity risk. Some medications alter taste perception affecting diet and nutrition. Antipsychotics and mood stabilizers can cause movement disorders affecting ability to maintain oral hygiene or increased sugar cravings. We review all medications with patients and caregivers, coordinate with prescribing physicians about oral health impacts, recommend strategies managing side effects like artificial saliva for dry mouth or specialized cleaning techniques for gum overgrowth, adjust preventive care frequency based on medication-related risks, and monitor for oral manifestations requiring intervention. Never stop medications without physician consultation, but understanding their oral effects allows targeted prevention.
Yes, numerous adaptive products help patients with limited dexterity maintain oral hygiene. Electric toothbrushes require less manual control and provide effective cleaning with minimal effort. Toothbrush handle modifications including foam grip enlargements, angled handles for better reach, or extended handles for caregivers assisting from behind make brushing easier. Three-sided toothbrushes surround teeth cleaning multiple surfaces simultaneously reducing brushing time and complexity. Floss holders and interdental brushes simplify cleaning between teeth for patients unable to manipulate traditional floss. Suction toothbrushes remove excess saliva and toothpaste for patients with swallowing difficulties. Mouth props help patients who cannot maintain open mouth position. We provide demonstrations with adaptive equipment, recommend specific products based on individual needs and abilities, offer samples for trial before purchase, and teach caregivers proper techniques. Many products are available through specialty medical suppliers or adaptive equipment vendors we can recommend.
We use proactive strategies preventing most outbursts through sensory accommodations, predictable routines, clear communication, and respecting each patient’s limits. When outbursts occur despite prevention efforts, our team remains calm and uses de-escalation techniques including reducing sensory stimulation by dimming lights and minimizing noise, giving the patient space and time to self-regulate, using calming sensory tools like weighted blankets or fidget items, and employing familiar calming strategies caregivers have shared. We never restrain patients except when absolutely necessary for immediate safety using only protective stabilization with caregiver consent. If a patient cannot safely continue, we stop treatment and reschedule rather than forcing cooperation which damages trust. We analyze what triggered the outburst to modify future approaches. Our goal is creating positive experiences even when treatment cannot be completed. Many patients who initially have frequent outbursts eventually tolerate care as trust builds through patient, respectful repeated exposure.
Biological dentistry is especially important for special care patients who often have compromised immune systems, multiple chemical sensitivities, or complex medication regimens making them more vulnerable to toxic exposures. We use biocompatible, toxin-free materials tested for safety in immunocompromised patients, avoid mercury, fluoride, BPA, and other potentially harmful substances, minimize exposure to cleaning chemicals and fragrances that may trigger behavioral or physiological responses in sensitive individuals, and employ gentle minimally invasive techniques reducing physical and emotional stress. We coordinate all materials and medications with physicians preventing dangerous interactions, review medication lists for oral health impacts, and adapt treatment protocols for conditions like bleeding disorders or immune suppression. Our whole-body health approach recognizes that oral health significantly impacts overall wellbeing especially for individuals with complex medical needs. This natural, holistic philosophy aligns perfectly with special care dentistry’s patient-centered focus on individualized, respectful treatment supporting each person’s unique needs.
We coordinate extensively with your healthcare team for comprehensive integrated care. We request medical histories and current medication lists, consult with neurologists about seizure management and medications affecting oral health, work with developmental pediatricians regarding behavioral strategies and developmental considerations, collaborate with behavioral therapists to align dental approaches with existing behavioral plans, coordinate with occupational and physical therapists about positioning and sensory strategies, and communicate with primary care physicians about overall medical status and clearance for procedures. For medically complex patients, we review medications for oral health impacts and interactions, coordinate timing of dental procedures with other medical treatments, adapt protocols for bleeding disorders or immune suppression requiring special precautions, and monitor for oral manifestations of systemic diseases. We provide comprehensive documentation for insurance authorization, maintain ongoing communication about changes in health status, and participate in care team meetings when needed ensuring all providers work together toward common goals.
